Annual Physicals Are Required For Sports

All students participating in LCISD sports and activities such as football, basketball, volleyball, drill team, color guard, marching band, and SAC camp must have an annual physical on file before they can participate in any of the activity, including clinics and try-outs. The physical must be dated after May 1 to be valid for the following year. This includes students who recently participated in Cheer try-outs. These students must also have physical dated after May 1 even though they might have just submitted one for try-outs.
 

Physicals at Foster HS on May 4

Although physicals can be completed by a physician of your choice, Foster High School Sports Medicine has arranged for Memorial Hermann/UT Physicians Medical Group to be at Foster Hight School on May 4, 2026 for a cost of only $20.

Pre-registration is required.

Students received a physical packet through their P.E. class recently. (A copy of the packet is attached to this notice.)

Be sure to read through the packet carefully.

Pre-registration forms must be turned in to the Foster Athletic Trainers at the Foster Fieldhouse no later than April 30th at 4:00 p.m.

Forms will NOT be accepted in the front office of Wertheimer or Foster.

Online payment must be made ONLINE by midnight May 1st. (Paying online is the ONLY option for payment.)

The Wertheimer Permission form must be turned in to Coach McDonald during the student's P.E. period no later than May 1st.

NO EXCEPTIONS WILL BE MADE TO THE INFORMATION ABOVE.

Recap

  1. Complete the pre-registration forms and turn them in to the Foster High School Athletic Trainers at the Foster Fieldhouse no later than April 30, 2026.
  2. Make the required online payment no later than May 1, 2026.
  3. Return Wertheimer Permission form to Coach McDonald during the student's P.E. period no later than May 1, 2026.
  4. On May 4th, Physical Day, make sure someone will be at Foster High School (Gym) to wait for your child to complete the physical. (No bus transportation after physical.)
